Tonsillectomy (Tonsillotomy and Coblation)

چکیده
Most tonsillectomies are carried out by dissection. Only a small minority of otolaryngologistsstill routinely perform guillotine tonsillectomy. We carried out a prospective study on 86children undergoing tonsillectomy utilising a standard anaesthetic and analgesic regimen tocompare post-operative pain after dissection tonsillectomy and guillotine tonsillectomy using aPopper's hemostatic guillotine. Guillotine tonsillectomy was significantly less painful(P<0.001) than dissection tonsillectomy. The relative risk of experiencing moderately severeto severe pain was 0.36 (95% CI, 0.18-0.72) in the guillotine group. A significant proportion ofchildren experience moderately severe to severe pain despite a comprehensive analgesicregimen confirming that post operative pain remains an important issue after this operation. On the basis of our findings we advocate tonsillectomy by guillotine in children. The less painthat arises within the first 24 h may be particularly important if performing tonsillectomy as aday-case procedure.PMID: 10699236 [PubMed indexed for MEDLINE] Clin Otolaryngol Allied Sci. 1997 Dec;22(6):532-5. [Coblation tonsillectomy versus blunt dissection tonsillectomy].
OBJECTIVE Compare coblation tonsillectomy with blunt dissection tonsillectomy to see if there is any difference in intraoperative bleeding, operating time, postoperative pain, tonsillar fossae healing and return to normal diet and activities. METHOD A randomised control trial was carried out to compare coblation tonsillectomy with blunt dissection tonsillectomy in 56 patients. The same surgeo...
متن کاملTonsillectomy versus Tonsillotomy for Sleep-Disordered Breathing in Children: A Meta Analysis
OBJECTIVES Tonsillotomy has gained popular acceptance as an alternative to the traditional tonsillectomy in the management of sleep-disordered breathing in children. Many studies have evaluated the outcomes of the two techniques, but uncertainty remains with regard to the efficacy and complications of tonsillotomy versus a traditional tonsillectomy. This study was designed to investigate the ef...
متن کاملEffort to engage Magnification Devices in Educational Tonsillectomy - A prospective clinical trial
Tonsillectomy is still one of the most common surgical procedures in otorhinolaryngology [1]. However, over the last decades, its incidence has constantly been decreasing due to tighter indication as well as the development of alternative surgical procedures like tonsillotomy [2]. This trend mainly relies on the risks and complications which are associated with tonsillectomy: namely post-operat...
متن کاملRe: Increased post-operative haemorrhage seen in adult coblation tonsillectomy.
Coblation is a new soft tissue surgical technique that is being used for tonsillectomy. Published results show a significant decrease in the amount of post-operative pain experienced by patients undergoing coblation tonsillectomy. There has been no published work to date on the incidence of post-operative haemorrhage. From August 2001 to November 2002 one surgeon performed 36 coblation tonsille...
متن کاملCoblation vs. Electrocautery Tonsillectomy: A Prospective Randomized Study Comparing Clinical Outcomes in Adolescents and Adults
OBJECTIVES Coblation is operated in low temperature, so it is proposed that tonsillectomy with coblation involves less postoperative pain and allows accelerated healing of the tonsillar fossae compared with other methods involving heat driven processes. However, the results of the previous studies showed that the effect of coblation tonsillectomy has been equivocal in terms of postoperative pai...
متن کاملCoblation tonsillectomy versus dissection tonsillectomy: a comparison of intraoperative time, intraoperative blood loss and post-operative pain.
The objective of this study was to compare the intraoperative time, intraoperative blood loss and post operative pain between coblation tonsillectomy and cold tonsillectomy in the same patient. A prospective single blind control trial was carried out on 34 patients whom underwent tonsillectomy. The patients with known bleeding disorder, history of unilateral peritonsillar abscess and unilateral...
